I CALL ON THE PHONE ALL MORNING AND THEY DON'T GET ME. I TRY TO ASK FOR AN APPOINTMENT ON THE INTERNET AND WILL NOT LEAVE ME BECAUSE IT IS ALL BUSY. I GO TO THE STREET DATA AND THEY WILL NOT LET ME IN .... WHAT DO I DO ????. THEN THEY DO NOT SAY THE DOCUMENTATION SUBMISSION TIME HAS EXPIRED.

The person who took care of collecting my request refused to let me go to work and told me that when they submitted that request they took care of everything until the doctor saw me. Well now it turns out that I owe 19 days to my company. AMAZING!
