Improve the ethics / courtesy of assistants in service to patients.

BPJS doctors who almost never have a place when patients seek treatment. if the patient goes for treatment, the assistant checks the patient's blood pressure and will ask for the patient's complaint, then the assistant will call the doctor and tell the patient's complaint via telephone. then the doctor will suggest what medication is in accordance with the patient's complaint. for the need for a letter of referral to the hospital, the same is done by the assistant. if approved by the doctor, the assistant will print a referral letter, which they have previously prepared a blank form that the doctor has signed for the benefit of the referral letter. what can be said as a doctor? this incident has been years experienced by patients. I am confused about where to report, because it is not in accordance with the ethics of a doctor.
