Sharing a personal experience, had taken all forms mentioned online in various forums for surrender of policy. Revenue stamp is required to be pasted and signed on the surrender form. In case you are visiting any lic branch for policy closure then at minimum carry the policy document in original, ADHAR and pan card in original and along with self attested copies, cancelled cheque which has ur name printed on it.nThough a mam(agent) in the branch was very helpful and made a revenue stamp available for me there itself else I would have to go near check naka to buy one.
