Blessed in 1691, it has the image on the main altar, adorned with a gilded wooden sunburst by G.F. Ferri, of the "Madonna and Child" (which tradition has found on the branches of an elm), to which devotees offered numerous votive tablets, once exposed on the walls. The four Sibyls and the stucco ornaments are by the Roman G.F. Guerniero. The two canvases of the side altars are from the Marche school of the late 1600s.
