It's normally done over the counter. You can also get meals and drugs for your pets, fresh bread, wine, juice, body and face creams and oils, body soaps, Milo, and a thousand other items! Don't lose your receipt after paying for an item, you have to submit it to the clerk besides the door before you can be allowed to leave.
