The Monument to the Chief is located on the eastern edge of Kościuszko Square in Tomaszów, formerly known as St. Joseph. Next to it, on the north side, there is one of the oldest tenement houses in Tomaszów, former seat of the magistrate. The monument was unveiled on November 3, 1918, nine days before the disarmament of the German garrison in Tomaszów Mazowiecki. The monument is made of red sandstone from Szydłowiec, it consists of a high, four-step obelisk plinth mounted on a stepped pedestal. It is crowned with a full-figure sculpture of the Governor, dressed in a suit, standing without a hat, facing east. On the front wall of the high plinth, on the coat of arms, there is an eagle from the Jagiellonian dynasty. On the back of the pedestal there is the inscription "Destroyed by the Nazi hordes in 1940. Rebuilt by the society of the city of Tomaszów Mazowiecki in 1947."
The monument was carved in sandstone. It was officially unveiled in October 1917, on the hundredth anniversary of Tadeusz Kościuszko's death. The monument is located in the city center, at the intersection of Mościckiego, Warszawska and Piłsudskiego Streets.
Behind the monument, there is a poplar avenue that leads to the square named after Tadeusz Kościuszko, which is the central square of Tomaszów Mazowiecki.
