They can not even make a health certificate in one week but my wife's visa will be expired 3 three weeks later.

While not the biggest hospital, it is not that bad overall. Many of the ER and nursing staff don't speak English and after 12 years here my Chinese is not that great either. It can be a bit of a problem at times I will admit.
