Sonoma Post Acute

I recently visited this facility during their Christmas holiday party and was very impressed! The staff were all extremely friendly and attentive to their patients and all the residents seemed well taken care of and happy. The facility had a great overall feel!

I have worked as a agency nurse from Portland OR to the Bay area for 3.5 years, and I have seen many facilities, some good, most bad, a few great. Sonoma Post Acute is among the great. The staff, especially the CNA staff, have mostly been in this building for many years and are very dedicated to their work and patients. Some of the nurses have also been there for many years and the caring that is felt is great. The facility is clean. The activities are many. The therapy department works hard to send people home quickly. I give our facility 5 ud83cudf1f.

My sister has been a patient for 3 1/2 years at this facility. Every time I visit my sister, I find the facility well-maintained, the staff is very professional and caring, and there are many activities provided for the patients during the day and night. If there is a health issue or a change in medication, I get a call from a nurse which is comforting since I live out of town. Quarterly conference call reviews are very helpful since the department heads are available to update me on the current health status of my sister and to answer questions. I highly recommend this facility.

I went to visit a good friend here and It nice to see how the staff communicate so well with the residents, As well as one day in particular it was halloween and staff really made it festive with their costume party for staff and residents, they even broke into an impromptu dance show, making it fun and memorable for all who attended.

Wonderful facility, I love the staff! Very knowledgeable, personable and attentive to their residents. I strongly recommend!
