I highly recommend Dr. Jon. I began seeing him for a shoulder injury and could not be happier with the progress. Dr. Jon understands how important it is to me to continue to be active, and he helps me with modifications for my workouts, so that I can continue my active lifestyle. He truly cares about his patients, always asks how I am feeling, and goes above and beyond. He has helped me create a plan with personalized home exercises for the time in between PT sessions, and I always feel that my PT sessions help my injury. Scheduling is also so easy and Dr. Jon is responsive and easy to communicate with. I have had a great experience and will continue to go to Dr. Jon for any PT that I need.

Jon is a great PT! He gives you his undivided attention for an entire hour, customizes your exercise routines to you specifically, and does manual work every session. Jon always also asks how you are feeling before beginning each session and truly cares about how you are feeling each visit. For anyone looking for a PT, I highly recommend Jon at The Form Fix.

Recently when I found out I tore my ACL skiing, there was no doubt in my mind who to call for my PT rehab after surgery- Dr. Jon at the Form Fix. Physical fitness is a lifelong passion of mine; having competed in collegiate athletics, run marathons, spartan races, etc. I love to be challenged. I knew that Dr. Jon would once again be the perfect fit for me and my post-surgery rehab. Dr. Jon’s 60 minute, one-on-one sessions, are tailored to your individual needs providing the perfect balance of cautious rehabilitation and muscular challenge. I saw Dr. Jon a few years ago for a hip injury. I went into Dr. Jon’s office that day discouraged, not being able to walk, and came out walking with a whole list of alternative, safe exercises for me to stay active as I recovered. Having previously been to multiple physical therapists throughout my days as a college athlete, the experience in Dr. Jon’s office stood out as refreshingly individualized, personalized, warm, and welcoming. If you’re looking for a physical therapist who is able to meet your specific rehabilitation needs, while keeping you on track with your personal fitness goals, all while providing a comfortable, relaxed, and fun environment, then I highly recommend Dr. Jon at the Form Fix.

I'm in my late fifties and try to work out fairly regularly to stay as healthy as I can. Years of incorrect form and age have left me with a very painful lower back and right oblique. I actually began to limp and my daily life was living with different levels of pain. Continuing and active lifestyle was severely hampered. A coach recommended Dr Jon. Dr Jon impressed me right away with his communication and detailed break downs to target the problem areas. The one on one sessions were crucial because you get his undivided attention. I was able to continue to workout as I rehabed. The process included everything from form breakdowns, stretching routines, massage and most importantly, communication. Dr Jon was available whenever I reached out to him in between our sessions. I highly recommend Dr Jon.

I am 50 years old and I work out fairly regularly (about 5 times a week). A few months ago I injured my distal bicep tendon while putting down a heavy weight. After giving it a rest for a few weeks I went right back to my usual routine and injured it further. I had always avoided PT in the past because I didn't understand or appreciate the value of it...but now I certainly do. I started with Dr. Jon after my re-injury and he methodically worked the injured area along with other areas that were contributing to the issue (ones which I never would have even imagined could be related). Between the in-person visits and the at-home exercises, I was able to restart some my former exercises within 3 weeks and all of them by 6 weeks. A truly remarkable recovery. I can't recommend him enough.
