My Father spent almost 2 months at this facility. The amount of care he got was exceptional! The stories i read online made me question my choice at first but the nurses that helped him daily and the interaction he got from PT, OT and Speech were amazing! They interacted with him and included him with all the activities they plan on a daily basis! I'll gladly give them a great review in person and online!
My sister was recently at Evergreen. They took very good care of my sister in her last days. She had physical therapy twice a day during the week. The ladies doing the physical therapy really worked with her. The nursing staff were very responsive to her needs. The entire staff at Evergreen was kind and courteous to my sister during her stay. Thank you to the entire staff.
ZERO is to good of a number, I beg you please do not send your loved one to that HORRIBLE nursing home. They are uncaring, ill-mannered, rude, rough, nasty and they will lie about your loved one and the care they are providing. Unfortunately my mother spend 2 months and we had to go daily to assure she was receiving proper care......which we found every day that she was not. Twice they ripped her feeding tube out of her stomach requiring surgery to replace. At night they left her on the bed pan for 45 minutes to an hour. Bandages were to be changed daily, they were changed every 3 days. It was so bad the doctor and the PA both called multiple times to inform them about what was needed for her care.....Evergreen DID NOT CARE. She was discharged last Wednesday and they have yet to get her prescriptions straighten out and they again DO NOT CARE. HORRRRRIBLE CUSTOMER SERVICE, please please if you love your family do NOT take them there!!!!!!!!

DO NOT SEND YOUR LOVED ONE HERE. Please, if you love them don’t choose this place! My dad lived here for 2 years before my sisters and I could get him into a new facility. At Evergreen, my dad would be in and out of the hospital several times a month. My sisters and I were so worried about him and his lack of care that we felt someone had to be there constantly checking on him daily. When my dad would become ill the staff would ignore it with out even notifying us (we would have to find out for our selves after being unable to get in contact with him for long periods of time or by showing up and finding him out cold or struggling to not choke on his own vomit). My dad is fully coherent and an out going guy, yet the nursing staff refused to even so much as let us know when he was not doing well when you could clearly tell. My dad would be completely knocked out , unable to drink or eat for days and the people in charge of this place would completely ignore it. The aids seemed to care more than the nurses, but they were unable to make any important decisions. My dad would be admitted into the hospital completely out of it and dehydrated. We had +10 near death scares. The hospital admitted Evergreen was known for being a horrible facility. If it weren’t for my sisters and I constantly advocating for him, he would not be alive today. Care plan meetings every 3 weeks involved a group of evergreen staff members that would fight with us about my dads care when we would try to speak about the issues that were occurring. They told us that my dad needed to choose one doctor (dentist, foot doctor, kidney doctor, etc.) because they were not willing to provide him with the care he deserves. There were several times my father did not receive a shower for over a week sometimes even two weeks. When we would confront the issues the people in charge would always decide to blame my dads behavior. They acted as if an elderly man (a veteran) in pain was never allowed to stand up for him self or his care. The food was horrible and they were unwilling to follow my dads dietary restrictions. The PT staff and a few of the aids showed compassion, but a majority of the staff especially the ones in charge showed no compassion at all and would actually argue with my dad and with us. As a residents daughter, I would get panic attacks frequently as my father had many near death experiences due to his constant lack of care. “Discussing issues or working on issues” does not exist in this establishment. My dad has been in a new facility for about 6 months now and his physical and mental health has improved tremendously. During care plan meetings at the new facility,my sisters and I no longer have to feel like we have to fight for my dads care because the staff is willing and prepared to help improve my dads quality of life in any way they can. Evergreen would purposely give my dad medication like sleeping medication, for example (that he did not need and his kidneys could not process) just so they could shut him up and ignore him. Even when I was there I would have to go out into the hallway, search for a staff member and ask them to change my dads soiled pants and still my dad would wait over an hour. I would ask several times for something to be taken care of before it would actually happen and a lot of times I would get some kind of rude reply. This place was a complete and total nightmare and if we hadn’t moved my dad out of Evergreen, I do not think he would still be alive today. I have a whole laundry list of issues we had with this place, but I will leave it at this. My dad is 79 and has the potential to live many more years. THE BEST THING WE EVER DID WAS MOVE MY DAD OUT OF EVERGREEN SO THAT HIS LIFE COULD FINALLY IMPROVE.
